EXPRESSED WISHES UPON DEATH IN INDIA

voluntarily and without coercion.  I fully understand the contents of 
the document and the implications therein.  I hereby inform the trustees 
of Avatar Meher Baba Trust that in the event of my death in India, I 
wish to be cremated, preferably at Meherabad.


Here are the names and addresses and phone numbers of two of my 
relatives or friends to be notified in case of my death or serious illness.

The Trust need not delay the disposal of my body for longer than 24 hours.


I have informed the above mentioned persons of my wish to be cremated in 
India, and they have accepted my decision in this matter.

(This form is mandatory for legal purposes.  It must be filled out, 
except for signatures; then notarized with witnesses and yourself 
signing. Witnesses can be anyone on the premises at the time of 
signing.  Witnesses to your signature cannot be partial to your 
interests. Thus, your wife or husband, for example, cannot sign as a 
witness. They are most likely going to be your chosen representative. A 
passport sized photo of you must accompany this document at the time of 
signing. Please note that your choice of representative can be changed 
at any time.  It is up to you to update this document as needed. 
Finally, the notarized copy must be given to Craig Ruff at Avatar Meher 
Baba Trust Office, Ahmednagar, and a certified copy, provided by the 
notary, to Dr. Anne Moreigne at Meher Hospital.)
