Documents and Information for Your Personal Representative


Your appointed Representative (or Buddy) will need to be prepared in 
advance should you fall ill, have an accident or become otherwise 
incapacitated, and be unable to make decisions for yourself. Also, 
he/she will need to know what to do if you should die unexpectedly.


The information he/she will need is covered in the following checklist. 
Originals of documents should be completed and photocopies included with 
other pertinent medical/personal information. These should be discussed 
with your representative and kept in a safe place that is immediately 
accessible to him/her.


Checklist:

_____ Completed Form: “Expressed Wishes Upon Death in India”


_____ Completed Form: “Appointment of Representative and Living Will”


_____ Medical Insurance Papers and Information, if applicable.


_____ Life Insurance Papers and Information, if applicable.


_____ Will with name of Executor/Executrix, if applicable.


_____ Bank Account Information, Especially How to Access Money in Rupees 
and Foreign Currency, if Applicable.


_____ Information on Financial Resources other than Bank Accounts and 
How It Can Affect Options for Health Care


_____ Medical Information (In Writing), Including Current or Past 
Doctors (In India, or Elsewhere), Current Medications, Allergies, Major 
Diseases, Surgeries, etc.


_____ Other Personal Wishes or Concerns


_____ Extra Set of Keys to Room, Vehicles, Safe Box, etc.


The notarized copies of your “Expressed Wishes Upon Death in India” and 
“Appointment of Representative and Living Will” should be given to Craig 
Ruff at AMBPPCT Office, Ahmednagar, and certified copies of each 
document to Dr. Anne at Meher Hospital.
